The “Pathology Report” is a very important document that describes the Pathologist’s findings of the examination of tissue or other materials that were submitted by a patient’s physician.
Report Copies
Patients may print the Patient Request Form on this website, fill it out completely and fax or mail it to us.
Payment Information
Patient Representatives are available to assist patients Monday through Friday, 9:00 a.m. – 4:00 p.m.
