The "Pathology Report" is a very important document that describes the Pathologist's findings of the examination of tissue or other materials that were submitted by a patient's physician.

Report Copies

Patients may print the Patient Request Form on this Web site, fill it out completely and fax or mail it to us.

Payment Information

MicroPath Laboratories will immediately bill the listed insurance carrier. Patient Representatives are available to assist patients Monday through Friday, 9:00 a.m. – 4:00 p.m.
